Purposeful Pathways Inc. will bill your insurance for sessions attended. You may be responsible for a deductible, coinsurance or copayment fee, as indicated by your insurance company. All no-shows and late-cancellations will be charged directly to the client as outlined in the Service Agreement.

*Family Therapy with Licensed Therapists - Due to low reimbursement rates from insurance companies, family therapy sessions with licensed therapists will require a private pay agreement.

If Purposeful Pathways Inc. is not contracted with your insurance, you may be eligible for “out-of-network” reimbursement from your insurance. It is recommended that you contact your insurance company and ask the following questions:

• Do I have mental health coverage? 
• What percentage of my bill will be reimbursed for services obtained by an out-of-network provider? 
• Is there a limit to how many sessions would be covered or how much would be reimbursed per calendar year? 
• Do I have a deductible? If so, how much remains before I would be eligible for out-of-network reimbursement?
• How long do I have to submit my request for reimbursement?

If you elect to use your out-of-network benefits, you would be responsible for paying for your sessions at the time service is rendered. Your provider would send you a receipt of services (aka a Superbill) that you would provide to your insurance company.
